WHAT IT IS, IN A SNAPSHOT:
- Video contest, open to all PHCA member employees, residents, their families and volunteers
- For long-term care employees and volunteers: Make a video that helps others see the value of the care that your facility offers. It could be a story about an unsung hero, about why you love your job, about how long-term care facilities are a place where the elderly can still live full lives, about lessons you’ve learned from your residents, about a successful program at your facility, about a challenge you face at your home that might be resolved with more adequate funding – any story you think is worth sharing. The goal here is to show legislators, public policymakers, the media, and the public that long-term care facilities are very much a place where people continue to live their lives, and that staff are focused on preserving the quality of life of the residents.
- For residents and their families: Make a video that shows the human side of the facility, and how it is providing quality care and quality of life for you or your loved one.
- Videos will be used in our grassroots efforts: We can send them to legislators, launch our own YouTube channel, put them on our Web site, share them with media, upload to CNN IReports, showcase at convention, etc.
PHCA/CALM Video Contest
Official Contest Rules
- Contest Eligibility: Entrants must be employees, residents, volunteers or family/friends of residents at a PHCA/CALM nursing home, assisted living residence or personal care home.
- Permissions. Before embarking on a video project, participants must obtain permission from the facility’s administrator. To ensure permission has been granted, videos must be submitted by the facility’s administrator.
- Entry Limits. Individuals or teams of individuals may submit up to three (3) videos.
- Length of Video. Videos should not exceed three (3) minutes.
- Rights. All submitted videos become the property of PHCA/CALM and can be used for promotional purposes that advance the mission of the association and the long-term care profession. Videos may be used for grassroots purposes as soon as they are received by PHCA/CALM.
- Media Consent. Before filming any residents, video contest participants must obtain a signed media consent form. If your facility does not have one, PHCA//CALM can provide one to your administrator. (Click here for the PHCA/CALM media consent form)
- Submission Timeline. Submissions will be accepted from Sept. 17, 2009 through Feb. 2, 2010. All submissions should be uploaded to the Web site, http://phca.illuminage.com/, maintained by our co-sponsor, IlluminAge Communications Partners.
- Format: Videos may be submitted in any of the following formats: Windows Media Video, MPEG, QuickTime, FLV or AVI.
- Judges. Winners will be chosen by a panel of judges, including IlluminAge staff, legislators, reporters, and others as chosen by PHCA/CALM.
- Announcement of Winners. Winners will be announced in February 2010 through emails and postings on our Web site.
- Prizes. One Grand Prize winner and four Runner-Up winners will be announced.
The Grand Prize Winner will receive $500 cash from PHCA/CALM and the
winner’s facility will also receive $1,000 in free Web/communications
services from IlluminAge.
The four Runner-Up Prize winners will receive $250 cash each, and their facilities will each receive $500 cash for an event for their residents. - Only Original Works Accepted. All video submissions must consist only of the original creative work (video, script, photo images) of the submitting participant or team. Background music does not have to be original.
- Release of Liability. By participating in the Contest, you agree to release and hold harmless PHCA/CALM and IlluminAge from and against any claim, loss, damage, liability, expense, impairment or cause of action related to your participation in the contest or your receipt of any prize.
- Withdrawing Video. Should you wish to have your Video removed from the contest, please let us know by contacting Alison Everett at aeverett@phca.org or 717-221-7935.
